When should I use Xeloda?

Xeloda (capecitabine) is a drug with antitumor effect. It is used in case of breast cancer (monotherapy of cancer in case of ineffectiveness or resistance to chemotherapy), gastric cancer (stomach cancer), colorectal cancer (metastatic colorectal cancer).

What you should know before taking Xeloda

Xeloda absolute contraindications: DPD deficiency, severe renal failure, hypersensitivity to one of the components, combination treatment with sorivudine and its structural analogues (such as brivudine), pregnancy and lactation, hypersensitivity to fluorouracil.

Other contraindications (Xeloda is prescribed with caution): ischemic heart disease, renal failure of moderate severity, liver failure, lactose intolerance, lactase deficiency, glucose-galactose malabsorption, combination treatment with coumarinic oral anticoagulants, elderly patients.

How should I use the drug?

Xeloda should be taken with water, preferably no later than half an hour after meals.

Monotherapy of colorectal cancer, breast cancer and colon cancer: Xeloda is usually administered 2 times a day (in the morning and in the evening) at a dose of 1250 mg/m2 (2500 mg/m2 per day). Make a 7-day break after 2 weeks of therapy.

The following dosing regimen is prescribed for Xeloda with other drugs: - Breast cancer: 2 times a day, 1250 mg/m2 (make a 7-day break after 2 weeks of therapy). Use concomitantly with docetaxel (intravenous infusion). - Colorectal cancer and stomach cancer: 800-1000 mg/m2 2 times a day for 14 days followed by a 7-day break, or continuously at a dose of 625 mg/m2 2 times a day. The recommended duration of the course for colon cancer is 6 months.

Xeloda as part of combination treatment: - Combination with cisplatin: 1000 mg/m2 2 times a day for 14 days followed by a 7-day break. Cisplatin in the form of intravenous infusion - 80 mg/m2 once every 21 days. - Combination with bevacizumab or oxaliplatin: 1000 mg/m2 2 times a day for 14 days followed by a 7-day break. Bevacizumab in the form of intravenous infusion - 7.5 mg/kg once every 21 days. Oxaliplatin in the form of intravenous infusion - 130 mg/m2 after bevacizumab. - Combination with irinotecan: 1000 mg/m2 2 times a day for 14 days followed by a 7-day break. Irinotecan in the form of intravenous infusion - 250 mg/m2 once every 21 days. - Combination with bevacizumab and irinotecan: 800 mg/m2 2 times a day for 14 days followed by a 7-day break. Irinotecan in the form of intravenous infusion - 200 mg/m2 once every 21 days. Bevacizumab as an intravenous infusion - 7.5 mg/kg once every 21 days.

The therapy should be canceled or temporarily interrupted if there are signs of toxicity. You should notify your attending physician if unwanted events develop.

The initial dose should be reduced in patients with moderate renal failure. It should be borne in mind that severe adverse events in the elderly usually develop more often than in younger patients. In this regard, the initial dose of Xeloda should be reduced to 75% in patients aged 65.

What side effects may Xeloda cause?

The most common side effects of Xeloda include fever, stomatitis, vomiting, drowsiness, asthenia, fatigue, back pain, myalgia, arthralgia.

Digestive tract: ulcerative stomatitis, appetite disorders, pain in the epigastric and abdominal region, dry mouth, flatulence, colitis, gastritis, esophagitis, gastrointestinal bleeding. There are also cases of cholestatic hepatitis and liver failure.

Skin: dermatitis, erythema, dry skin, alopecia, skin itching, photosensitivity reactions, skin cracks, radiation dermatitis.

CNS: sleep disorders, paresthesia, headache, peripheral neuropathy, dizziness, encephalopathy, depression.

Respiratory system: shortness of breath, sore throat, cough.

Cardiovascular system: edema, angina pectoris, myocardial infarction, arrhythmia, heart failure, tachycardia, atrial fibrillation.

Blood: anemia, pancytopenia, neutropenia.

Infections: infectious complications associated with myelosuppression, systemic infections of various etiology (fungal, viral, bacterial), sepsis.

Xeloda special instructions

Patients with severe diarrhea should be carefully monitored. Standard antidiarrhoeal drugs should be prescribed as soon as possible. If necessary, the dose of the drug should be reduced.

Xeloda therapy may cause heart failure, angina pectoris, myocardial infarction, cardiac arrest, arrhythmias. The doses of Xeloda should be reduced in case of severe reactions.

Xeloda is not prescribed concomitantly with sorivudine.

Xeloda potentiates the effects of indirect anticoagulants. If you need to take these drugs at the same time, then you should adjust the dose of anticoagulants.

Xeloda increases the concentration of phenytoin.

My father who was diagnosed with stage 4 of colorectal cancer a little more than one year ago is taking this drug Xeloda. Sincerely this is a good drug that really helped my father and for that I’m very thankful now. Xeloda has had good effects in shrinking the tumor on his colon which doctors said that this would be extremely hard to do. we’ve been warned by the doctors that my father can have some of the following side effects: frequent vomiting, changed appetite and constant diarrhea, loose of hair and hair thinning and loss of tastes. However, he didn’t had any of them, he’s not vomiting, he is eating very well and he still taste the flavors of what he eats. I’m very thankful for this as well. It seemed that this drug helped me shrinking the tumor and without side effects but a little later we discovered some side effects that doctors didn’t warned us: he’s having pain on his hands and feet, he has chills and he’s feeling weak occasionally with body aches. Moreover, some times he is disoriented and I don’t know if the medication is responsible for this but his hearing isn’t as good as it used to be. Overall, I would give 80-90 from 100 to Xeloda. I just hope that it would continue helping my father in future.

I was on Xeloda for about 3 years now due to my cancer. I’ve developed cancer cells in my right lung and in my spine. Doctor said I have stage 4 of cancer. Sometimes I also have some symptoms in my hands and in my foot and because of this I cannot walk and my hands are in pains, are very sore and smooth. They are so painful and sore that I can’t hold a glass of water. Or even an empty glass. I was already thinking about death but then I thought that if my life didn’t stopped then I should live it for as long as I can. I try to do my best: I eat little meat, no coffee, sugar and other unhealthy foods. I eat as much vegetables and fruit as I can. Mostly of my diet is organic. So far, this product is good but I don’t know how much is going to be like that. Anyway, I’m thankful for all this time it gave. I guess it could be a great drug for a lower stage cancer. I have only 55 years old and I suppose that I’m too young… however I guess I already gained some time so… thanks for everything.
