Omnicef pharmacologic effect

Omnicef (cefdinir) is an oral cephalosporin (broad-spectrum antibiotic). This drug has become one of the most widely prescribed antibiotics for the treatment of pediatric infections.

Omnicef is approved for the treatment of pharyngitis or tonsillitis in children and adults, and also for the treatment of uncomplicated skin infections. It is also approved for use in pneumonia, acute exacerbations of chronic bronchitis and acute maxillary sinusitis in adolescents and adults.

Cefdinir is a semisynthetic cephalosporin, which is active against a number of gram-positive and gram-negative aerobic bacteria. Cefdinir shows activity against the following bacteria:

  • Gram-positive microorganisms (Staphylococcus aureus, Streptococcus pneumoniae, Streptococcus Pyogenes);
  • Gram-negative microorganisms (Haemophilus influenzae, Haemophilus parainfluenzae, Moraxella catarrhalis).

Contraindications and warnings

Cefdinir is contraindicated in patients with severe hypersensitivity reactions to cephalosporins.

Caution should be exercised when using cefdinir in patients with hypersensitivity to beta-lactam antibiotics.

Just like other antibiotics, cefdinir may affect the bacterial flora of the gastrointestinal tract and lead to diarrhea.

What should you know about Omnicef side effects?

Cefdinir therapy in adults can cause diarrhea (15% of patients), vaginal candidiasis (4% of patients), nausea (3% of patients), headache (2% of patients), abdominal pain (1% of patients), rash (0.9% of patients) and stomach discomfort or vomiting (0.7% of patients).

Similar side effects reported in pediatric trials: diarrhea (8% of patients), rash (3% of patients), vomiting (1% of patients), stomatitis (1% of patients), abdominal pain (0.8% of patients), vaginal candidiasis (0.3% of patients), and leukopenia (0.3% of patients).

Serious side effects: hypersensitivity reactions (including toxic epidermal necrolysis and Stevenson-Johnson syndrome), coagulation disorders (thrombocytopenia, idiopathic thrombocytopenic purpura, gastrointestinal bleeding, disseminated intravascular coagulation).

Nephrotoxicity and hepatotoxicity are associated with Omnicef.

Drug interactions

Cefdinir should not be taken together with antacids containing magnesium or aluminum.

Additives containing iron can reduce the absorption of cefdinir (from 30 to 80%). Patients treated simultaneously with cefdinir and preparations containing iron may have symptoms associated with stool color changes. If any change is accompanied by abdominal pain, diarrhea or fever, patients should discontinue treatment and immediately contact their health care provider.

It is not recommended to use probenecid during treatment with cefdinir. Probenecid violates the renal excretion of cefdinir.

Recommendations for dosing

The recommended dose for children from 6 months to 12 years of age is 7 mg/kg twice a day or 14 mg/kg once a day for 10 days (preferably in patients with acute otitis media or pharyngitis/tonsillitis).

Adolescents and adults with pneumonia or uncomplicated infections of the skin: the recommended dose is 300 mg twice a day for 10 days.

Acute maxillary sinusitis: the recommended dose is 300 mg twice daily or 600 mg once a day for 10 days.

Patients with pharyngitis/tonsillitis or chronic bronchitis: the recommended dose is 300 mg twice a day for 5-10 days or 600 mg once a day for 10 days.

Omnicef special instructions

Cefdinir may be used regardless of meals.

The drug is well distributed in tonsil tissue, sinuses, middle ear, lungs. The active substance is eliminated mainly via the kidneys.

Patients with transient or persistent renal dysfunction should reduce the daily dose of Omnicef.

Elderly patients do not need to change the dosage regimen.
