What should patients know about Livial?

Livial (tibolone) is a drug for the treatment of climacteric syndrome. Livial is used in postmenopausal women:
– Treatment of symptoms of estrogen deficiency;
– Prevention of osteoporosis in patients with high risk of fractures (prevention of osteoporosis).

What are the contraindications for Livial?

Livial has a number of contraindications. These include: breast cancer, estrogen-dependent malignant tumors (including endometrial cancer), untreated endometrial hyperplasia, vaginal bleeding of unknown etiology, thrombophilic conditions (including antithrombin deficiency), arterial or venous thrombosis, thromboembolism (including thrombophlebitis and deep vein thrombosis, myocardial infarction, ischemic or hemorrhagic cerebrovascular disorders, pulmonary embolism), angina, transient ischemic attacks and other conditions associated with thrombosis, uncontrolled hypertension, atrial fibrillation, subacute bacterial endocarditis, immobilization after surgical treatment, obesity and other multiple or severe risk factors for the emergence of arterial or venous thrombosis, severe liver disease or abnormal liver function, liver tumors (including benign liver tumors), liver failure, galactose intolerance, glucose-galactose malabsorption, lactase deficiency, pregnancy and breastfeeding, hypersensitivity to the drug.

Livial should be taken with caution in bronchial asthma, uterine fibroids and endometriosis, hypertension, cholelithiasis, symptoms of estrogen-dependent tumors, renal failure, high level of cholesterol, disorders of carbohydrate metabolism, diabetes, systemic lupus erythematosus, migraine or severe headache, endometrial hyperplasia, epilepsy, otosclerosis. Careful medical supervision is recommended if these symptoms get worse while taking Livial.

Livial dosage

Do not start taking the drug earlier than 12 months after the date of the last menstrual bleeding! Take Livial the next day after the completion of the previous treatment regimen (in case of previous hormone replacement therapy).

The tablets are taken whole with water. The usual dosage is one tablet per day. It should be taken every day at the same time. Pills in a blister pack correspond to the current day of the week. Start the new blister the next day after you have finished taking the pills from the previous one. Do not miss the next tablet or take double doses.

What are the side effects for Livial?

Potential adverse reactions may include:

Reproductive system: often – pain in the breast, vaginal bleeding, vulvovaginitis and vulvovaginal candidiasis, genital itching, pain in the pelvic area, cervical dysplasia.

Gastrointestinal tract: often – pain in the abdomen.

Skin and subcutaneous tissue: often – excessive hair growth; rarely – acne.

Other reactions: the frequency is unknown – weight gain, depression, headache, dizziness, pruritus and rash, seborrheic dermatitis, gastrointestinal disorders (bloating, diarrhea), visual disturbances (including blurred vision), peripheral edema, fluid retention, joint and muscle pain, functional disorders of the liver, elevated liver enzymes.

Therapy with preparations containing only estrogen or tibolone may be associated with increased risk of breast cancer, but it is much lower than in the case of therapy with combined hormonal agents. The degree of risk depends on the duration of therapy. Estrogen and progestin combination (when used over 5 years) increases the incidence of breast cancer. Livial increases the risk of endometrial cancer and ischemic stroke. Patients may suffer from venous thromboembolism, including deep vein thrombosis and pulmonary embolism (especially in the first year of therapy). Chronic administration of tibolone may lead to ovarian cancer. The drug can also lead to coronary heart disease (in women older than 60 years), myocardial infarction, cholecystitis, skin disorders (chloasma, erythema multiforme, vascular purpura), dementia (in women older than 65 years).

Livial special instructions

Livial is not used as a contraceptive method. Patients should take into account all risk factors and benefits of treatment. The doctor must inform you about all the risks associated with taking the drug (including the risk of getting cancer). Unwanted side effects (including cancer) are directly dependent on the duration of treatment and the patient’s age.

Patients must inform the doctor about the slightest changes in the mammary glands. Livial should be discontinued in case of contraindications, as well as jaundice, abnormal liver function, high blood pressure, severe headache (migraine).
