Imovane (zopiclone) mechanism of action

Imovane (zopiclone) is a drug used for the treatment of insomnia (acute, chronic and situational insomnia). This is a psychotropic agent with hypnotic, sedative, anxiolytic, and anticonvulsant action. Imovane is indicated for adults who have difficulty falling asleep.

Imovane reduces the number of nighttime awakenings and helps maintain a normal sleep cycle (typical eight-hour sleep).

Imovane (zopiclone) contraindications

Imovane (zopiclone) should not be given to patients with severe respiratory insufficiency, sleep apnea, severe hepatic impairment, myasthenia gravis, hypersensitivity to the drug. Imovane is not used in pediatric patients (up to 18 years of age). The drug is not recommended for use in lactating and pregnant women.

Imovane (zopiclone) dosage

Imovane tablets should be taken at bedtime. The therapy should last no longer than 1 month (if possible). The recommended adult daily dose is 7.5 mg. Do not use extra doses (excess is not recommended).
The average duration of treatment:
Transient insomnia – 2-5 days;
Situational insomnia – 14-21 days.

Elderly patients and patients with impaired liver function, renal failure, or chronic obstructive pulmonary disease should start the therapy with a dose of 3.75 mg. If necessary, the daily dose may be doubled.

Imovane (zopiclone) side effects

The most frequently observed adverse events associated with taking Imovane include bitter taste in the mouth.
Digestive system: dyspepsia, dry mouth, nausea; in rare cases – elevated liver enzymes.
Central nervous system: residual drowsiness, headache, dizziness, anterograde amnesia; rarely – depression, hallucinations, nightmares, confusion, irritability, aggressiveness, confusion, incoordination, depressed mood.
Allergic reactions: rash, itching; very rarely – anaphylactic reactions, angioedema.
Rapid discontinuation of therapy can cause withdrawal symptoms including symptoms such as tremors, irritability, hallucinations, anxiety, tremor, sweating, agitation, headache, confusion, delirium, tachycardia, nightmares; very rarely – seizures.

Imovane (zopiclone) overdose

Overdose may result in CNS depression: confusion, drowsiness, lethargy. There may be cases of sinus bradycardia, ataxia, hypotonia, hypotension, respiratory depression and coma.

Major risk factors for overdose: alcohol beverages and other central nervous system suppressors.

Imovane (zopiclone) warnings

Patients should remember that there is a small possibility of addiction to zopiclone. The risk is higher in case of disorders of dose and duration of therapy, drug abuse or alcohol abuse, etc. Long-term therapy may lead to withdrawal symptoms and rebound insomnia. As a result, the dose should be reduced gradually.

Imovane should not be used for the treatment of depression. Anterograde amnesia may occur during therapy, especially when sleep is interrupted. The risk is reduced while taking the drug just before bedtime (duration of sleep at least 6 hours).

Imovane may impair the ability to drive. Therefore, it is recommended to refrain from such activities during the whole period of treatment.

Imovane may cause unwanted effects when used together with some other drugs: erythromycin, neuroleptics, hypnotics, antiepileptic drugs, sedatives, tranquilizers, antidepressants, opioid analgesics, anesthetics, antihistamines with sedative effect.

Imovane may reduce the concentration and the effects of trimipramine. Ritonavir, clarithromycin, erythromycin, and ketoconazole may increase the concentration of zopiclone, while carbamazepine, rifampicin, phenobarbital, phenytoin may reduce zopiclone concentrations.

When used together with Imovane, clozapine can cause shock, respiratory arrest, cardiac abnormalities.
