Ilosone (erythromycin) indications for use

Ilosone (erythromycin) belongs to the group of macrolide antibiotics. It has bacteriostatic properties. However, it also has bactericidal activity against susceptible microorganisms (in high doses).

Erythromycin is active against gram-positive bacteria: Staphylococcus spp., Streptococcus spp. (including Streptococcus pneumoniae). Gram-negative bacteria: Haemophilus influenzae, Neisseria gonorrhoeae, Bordetella pertussis, Brucella spp., Legionella spp., Bacillus anthracis, Corynebacterium diphtheria. Anaerobic bacteria: Clostridium spp. Erythromycin is also active against Mycoplasma spp., Chlamydia spp.

Ilosone main indications: infections of the upper and lower respiratory tract (tonsillitis, otitis, sinusitis, tracheitis, bronchitis, pneumonia), biliary tract infections (cholecystitis), skin and soft tissue infections (pustular skin disorders including acne vulgaris, infected wounds, trophic ulcers), infections caused by Listeria monocytogenes, primary syphilis, diphtheria, whooping cough, erythrasma, intestinal amebiasis, chlamydia (neonatal conjunctivitis, pneumonia in children and genitourinary infection in pregnant women caused by Chlamydia trachomatis).

Ilosone may be used for prevention of bacterial endocarditis in patients with congenital heart disease during dental or surgical procedures ((including preoperative bowel preparation, dental procedures, endoscopy in patients with heart defects). It is also effective for the prevention of exacerbations of streptococcal infection (tonsillitis, pharyngitis) in patients with rheumatism. Erythromycin is used in patients who are allergic to penicillin.

What are the most common side effects of Ilosone (erythromycin)?

Ilosone side effects include: diarrheal disorders, abnormal liver function, cholestatic hepatitis, jaundice, arrhythmia, superinfection, allergic reactions.

Ilosone (erythromycin) dosage and administration

Adults: take 250 mg every 6 hours (up to 4g a day or more). Children: 30-50 mg/kg in divided doses.
Streptococcal infections: 20-50 mg/kg per day in divided doses. The course of the treatment in infections caused by group A streptococcus – not less than 10 days.
Primary syphilis: 20 g in divided doses for 10 days.
Amoebic dysentery in adults: 250 mg 4 times a day, children – 30-50 mg/kg per day for 10-14 days; whooping cough – 40-50 mg/kg per day for 5-14 days.
Neonatal conjunctivitis: 50 mg/kg in 4 divided doses daily for at least 2 weeks.
Pneumonia in children: 50 mg/kg in 4 divided doses per day for at least 3 weeks.
Urogenital infections during pregnancy: 500 mg 4 times a day for at least 7 days.
Prevention of bacterial endocarditis: 1 g (20 mg/kg for children) for 1 hour prior to the procedure, 500 mg (10 mg/kg for children) within 6 hours after the procedure.

Ilosone (erythromycin) overdose

Symptoms of overdose include nausea, vomiting, diarrhea, discomfort in the stomach, abnormal liver function, acute liver failure, hearing loss, tinnitus, dizziness (especially in patients with renal or hepatic insufficiency).
Treatment: activated carbon treatment, gastric lavage. Hemodialysis, peritoneal dialysis and forced diuresis are ineffective.

Ilosone (erythromycin) interaction with other drugs

Ilosone increases the concentration of theophylline and digoxin. It enhances the effect of oral anticoagulants, ergotamine, dihydroergotamine.

The drug increases the concentration of carbamazepine, cyclosporine, hexobarbital, phenytoin, disopyramide, lovastatin, and bromocriptine.

Special instructions for Ilosone

Use caution in patients with liver and kidney disorders.

Drugs that increase the acidity of gastric juice may negatively affect the gastrointestinal absorption of erythromycin. Do not drink milk and dairy products while taking erythromycin.

Erythromycin crosses the placental barrier. Pregnant women should evaluate the expected benefit and the potential risk to the fetus. Patients should stop breastfeeding during therapy with erythromycin.

Ilosone is contraindicated in case of jaundice, liver failure and other serious problems.
