What is Gleevec (imatinib)?

Gleevec (imatinib) is an antineoplastic agent indicated for the treatment of oncological diseases in adults: - As monotherapy in patients with recurrent or refractory acute lymphoblastic leukemia (ALL); - In combination with chemotherapy in newly diagnosed Ph+ ALL; - Systemic mastocytosis in patients negative for D816V KIT (or with unknown KIT mutational status); - Chronic eosinophilic leukemia or hypereosinophilic syndrome; - CD117-positive gastrointestinal stromal tumors (unresectable or metastatic GIST); - CD117-positive gastrointestinal stromal tumors (adjuvant therapy); - Dermatofibrosarcoma protuberans (recurrent, unresectable or metastatic DFSP).

In addition, Gleevec is prescribed for adults and children with Ph-Chromosome-positive chronic myeloid leukemia, chronic myeloid leukemia (accelerated phases and blast crisis).

When Gleevec (imatinib) is contraindicated?

Gleevec (imatinib) is contraindicated in pregnancy and breastfeeding, in patients younger than 2 years of age, in case of hypersensitivity to the drug. Gleevec is used with caution in patients with severe renal dysfunction, severe liver disease, cardiovascular disease or risk factors for heart failure, as well as in patients on regular hemodialysis treatment.

Gleevec (imatinib) dosage instructions

Gleevec (imatinib) is best taken with food, with at least 1 cup of water. If necessary, the capsules or tablets may be dissolved in water or apple juice. The suspension should be prepared immediately before use. The dose and treatment period should be determined by your doctor (based on clinical indications).

Doses of 400 and 600 mg are taken once a day; doses of 800 mg are taken in 2 divided doses (morning and evening). Systemic mastocytosis in patients negative for D816V KIT: 400 mg per day. Systemic mastocytosis with unknown KIT mutational status: 400 mg per day. Chronic myeloid leukemia: chronic phase - 400 mg per day; acceleration phase and blast crisis - 600 mg per day. The dose may be increased up to 800 mg in case of good tolerability and absence of neutropenia, thrombocytopenia or other unwanted effects. The maximum dose for children (2 years of age) is 600 mg per day. It can be taken once or in equal parts (2 times a day). Ph+ ALL: 600 mg per day. Myelodysplastic/myeloproliferative diseases: 400 mg per day. Gastrointestinal stromal tumors: 400 mg per day. The dose may be increased to 600 or 800 mg. Adjuvant therapy in gastrointestinal stromal tumors: 400 mg per day for 36 months or more. Dermatofibrosarcoma protuberans (recurrent, unresectable or metastatic DFSP): 800 mg per day. Receiving Imatinib lasts until the termination of the clinical effect. Patients with mild, moderate or severe form of functional disorders of the liver should take the drug in a daily dose of no more than 400 mg. The dose should be reduced in case of toxic side effects. The drug should be used with particular caution in patients with severe hepatic insufficiency. The doses for elderly patients should not be corrected. Temporary suspension of therapy is required in case of neutropenia and thrombocytopenia.

Side effects of Gleevec (imatinib)

The following side effects are associated with Gleevec (imatinib): Infectious diseases: rarely - sinusitis, nasopharyngitis, herpes simplex, pneumonia, urinary tract infections, fungal infections, polyps and cysts; Hematopoiesis: very often - thrombocytopenia, neutropenia, anemia; often - febrile neutropenia, pancytopenia; rarely - lymphopenia, thrombocythemia, lymphadenopathy, eosinophilia, hemolytic anemia; Metabolism: often - anorexia; rarely - hypokalemia, hypophosphatemia, hyperuricemia, hyponatremia, hyperglycemia, hypercalcemia, hypomagnesemia; Nervous system: very often - headache; often - taste disturbance, insomnia, dizziness, paresthesia; rarely - hemorrhagic stroke, peripheral neuropathy, somnolence, migraine, tremor, depression, decreased libido, anxiety, increased intracranial pressure, optic neuritis, seizures, confusion; Cardiovascular system: rarely - hot flashes, palpitations, tachycardia, congestive heart failure, hemorrhage, pulmonary edema, angina, arrhythmias, low blood pressure, atrial fibrillation, myocardial infarction, hypertension, Raynaud's syndrome, hypotension; Respiratory system: often - shortness of breath, nosebleeds, cough; rarely - pain in the throat or larynx, pharyngitis, pulmonary fibrosis, pulmonary hemorrhage, pulmonary hypertension; Digestive system: very often - indigestion, nausea, vomiting, abdominal pain, diarrhea; often - dry mouth, flatulence, constipation, GERD, gastritis, stomatitis, gastrointestinal bleeding, ulceration of the mucous membranes of the mouth, esophagitis, gastric ulcer, pancreatitis, dysphagia, hyperbilirubinemia, hepatitis, jaundice; rarely - colitis, inflammatory bowel disease, hepatic necrosis, hepatic failure; Dermatological reactions: very often - skin rash, dermatitis, eczema; often - dry skin, itching, swelling of the face, sleep hyperhidrosis, alopecia; rarely - bruises, pustular rash, ecchymosis, urticaria, skin hyperpigmentation or hypopigmentation, exfoliative dermatitis, hypotrichosis, folliculitis, psoriasis, angioedema, Stevens-Johnson syndrome, erythema multiforme; Musculoskeletal system: very often - myalgia, arthralgia, muscle cramps and spasms, bone pain; often - swelling of the joints; rarely - arthritis, muscle weakness; Urinary system: rarely - frequent urination, hematuria, acute renal failure; Reproductive system: rarely - erectile dysfunction, gynecomastia, menorrhagia, sexual dysfunction, menstrual disorders; Other reactions: often - edema, fluid retention, weight gain, fatigue, fever; rarely - chest pain.

Gleevec (imatinib) special instructions

Gleevec therapy may only be carried out under supervision of oncologist.

It is necessary to control hematological parameters and body weight (especially in elderly patients with cardiovascular disease).

I felt myself bad for many months. There were some months when I just couldn’t wake up from my bad because it was hard for me to walk. I started to take Gleevec on 2009 and after this I started to have some serious issues with my iron level that got low. When I first decided to do some exams they didn’t catch the level of iron on my blood. Just after some months my doctor found out that I have serious problems with the iron level. Now after a couple of years I have serious joints pain, feet cramping and other issues. During all this years I noticed that my doctors don’t care so much about my health. Even with the administration of Gleevec they weren’t so interested about how much should I take. So please before using something convince yourself that it’s the right thing to do.

A year and a month ago I’ve had a surgery in order to get my gastro intestinal stromal tumor removed. I didn’t wanted the surgery but there was nothing else I could do. After the surgery, doctor said that I should have a ct scan at least every 3 months or if I’m not feeling well and there are some complications. Luckily, no complications occurred. Right after the operation doctor gave me Gleevec and now, after a year after that surgery, I am completely clear and I don’t have any evidence of disease anymore and I’m very very happy. For all my happiness is responsible Gleevec! This is a very good drug that will help you and it will make you feel a lot better. I have only one side effect that can be noticed: I have nausea from time to time and this I don’t like it but compared to what this drug gave me instead, it’s nothing! I hope that soon I’ll be able to stop taking it and then all the pains and all the nausea I was once feeling would go away once and forever. Until now thanks to the one who created this medicine

I really don’t have enough words to describe this drug! It’s the best drug ever created and I would really want to shake hands with dr. Druker for inventing this awesome and wonderful drug! This drug has really saved my life and there’s nothing in this world better than it! I’m very sorry for whoever says that Gleevec doesn’t work for them. My recommendation is: take care if you are using it correctly because I used to exactly as it was said and now I’m the happiest man because it worked so well! I’m not in remission for few years and this drug worked for me instantly! When I was firstly diagnosed with Chronic Myelogenous Leukemia (CML) my white blood cells count was more than 200000 and within just one month of using this drug my white blood cells count was reduced by more than 90% and few months later doctor said that white count is completely undetectable! The side effects are too few to worry about, in fact I had only one very mild side effect: diarrhea, which is nothing compared to what effects this drug had. Later I even found out that I can treat well the diarrhea with Imodium and it became not a problem at all. That’s the only “disturbing” thing I have ever had due to Gleevec and it’s too little for my attention. Life became great since starting the treatment and I’m down to two tablets a day. We should learn hoping for a better life and finding that something for making it real! I found it!

Gleevec seemed to be a good medication for me at first but then it just stopped working for me. It was a really good thing against my chronic myelogenous leukemia that was diagnosed a little more than one year ago. Right after the diagnose, doctor put me on gleevec. First few times I had some side effects such as rash all over my body but for one or another reason they subdued and over time I had no side effects and the effects were great. Recently, my bnr went from 3 to 18 and we suppose that my body build up antibodies for this medication so that’s why it stopped working. Tomorrow doctor is going to prescribe me my next medication.

For those who are suffering from chronic myelogenous leukemia I recommend this Gleevec drug! I’m suffering from this horrible disease and honestly I thought that by this time I won’t be alive. My doctor said from the very beginning that Gleevec should help me. I was dubious. On September 2008 I started my treatment with this medicine. I had white blood cells count 320.000+ in September 2008. In the beginning of January 2009 I had less than 50.000 white blood cells count. I was fully treated into full molecular remission within a year of taking this drug. In the beginning of 2010 I already forgot that I ever had such a disease. This medicine has literally saved my life! Doc said that I can choose to have a transplant surgery or I can start this treatment. I’m so happy that I choose the second variant. Chances of survival were less than 50% for transplant surgery. Now I have no more Philadelphia Chromosome since I got rid of the disease. I have a BCR-ABL blood test every half a year and the results are great each time. I have minor to no side effect at all. I can mention only vimiting and nauseous feeling but the benefits greatly outweigh the side effects. I hope this medicine will help everybody as it helped me.
