What is Detrol (tolterodine)

Detrol (tolterodine) is a drug used in the treatment of urological diseases. Tolterodine has antispasmodic action. It reduces the contractile function of the bladder and the secretion of saliva. Tolterodine increases the amount of residual urine and lowers detrusor pressure. It is widely used to treat overactive bladder (frequent urination or urinary incontinence).

How to use Detrol (tolterodine)

Tolterodine should be taken at a dose of 2 mg twice a day. The dose should be reduced in the presence of hepatic or renal insufficiency or severe side effects (1 mg twice a day). The duration of treatment may range from three to six months.

Detrol (tolterodine) contraindications

Tolterodine is contraindicated in case of hypersensitivity, myasthenia gravis, angle-closure glaucoma, severe ulcerative colitis and megacolon.

Use with caution in case of urinary retention (severe obstruction of the lower urinary tract), decreased intestinal motility, diseases of the gastrointestinal tract such as pyloric stenosis, renal or hepatic impairment (the daily dose should not exceed 2 mg), neuropathy, hiatal hernia. Use with caution in patients under 18 years.

There are no adequate and well-controlled studies on Detrol safety in pregnancy. Also, Detrol may cause allergic reactions, flu-like symptoms, weight gain, sinusitis, bronchitis and skin dryness. Common reactions: dryness of the oral mucosa, headache, constipation, dizziness, abdominal pain, dyspepsia, fatigue, drowsiness, visual disturbances, flatulence, dysuria, vomiting, fatigue, nervousness. Rarely: chest pain, dry skin, bronchitis, weight gain, sinusitis, urinary retention, confusion, gastroesophageal reflux, allergic reactions, angioedema, tachycardia, palpitations, peripheral edema, hallucinations. Nervous system: xerophthalmia, blurred vision, headache, dizziness. Cardiovascular system: hypertension. Gastrointestinal tract: nausea, dry mouth, diarrhea, flatulence, abdominal pain, constipation. Urogenital system: urinary tract infections.

Detrol (tolterodine) overdose

The most important overdose symptoms: disturbance of accommodation, difficulty with urination, hallucinations, severe agitation, convulsions, respiratory failure, tachycardia, urinary retention, dilation of the pupils. Treatment: gastric lavage and activated charcoal.

Detrol (tolterodine) interaction

Anticholinergic drugs enhance the therapeutic effect of the drug and increase the risk of side effects. Muscarinic receptor agonists may reduce the effect of tolterodine.

Avoid co-administration with certain inhibitors of CYP3A4, such as macrolide antibiotics (erythromycin and clarithromycin) or antifungal medicines (ketoconazole, itraconazole and miconazole).

Detrol has no harmful interactions with warfarin and combined oral contraceptives (containing ethinylestradiol/levonorgestrel).

Detrol (tolterodine) special instructions

Women of reproductive age should start Detrol therapy only if effective contraception is used.

Be particularly careful when using the drug in patients who take certain antiarrhythmic drugs (quinidine, procainamide) or class III antiarrhythmic drugs (amiodarone, sotalol).
